• Hisense product code, valid serial number, purchase date, retailer, and a copy of proof of purchase*.
  
  

• *Proof of purchase must be in the form of a copy of a valid receipt for the purchase of a Qualifying Product from a Participating Retailer during the Promotion Purchase Period and must include Participating Retailer name, Qualifying Product model code or name, date of purchase, and purchase price.
  
  

• Your bank account information including Account number and Sort code.
